This is the difference between a plugin and a hack. We use documented hooks and filters, follow the WordPress coding standards, and never patch core. Our own commercial plugins have to pass this test on thousands of sites.

Usually that is the right answer, and usually what we do. Extending a well-maintained platform costs less to build and far less to own than rewriting it.

Yes, after a paid audit. The audit tells you honestly what is salvageable. Sometimes it is most of it; sometimes finishing costs more than restarting, and you should know which before you commit.

The written technical scope. It defines the data model, the admin surfaces and the acceptance criteria before any code is written, and the price is fixed against it. Changes are priced as changes, visibly.
